Consent to provide personal data

1) The controller and processor of personal data is Everest Ayurveda, s.r.o. registered office: Bystrá 761/10, 19300 Prague 9, Czech Republic.

Pursuant to Act No. 18/2018, On Personal Data Protection, as amended (hereinafter also referred to as the “Personal Data Protection Act”), and in accordance with Article 6 (1) (a) | (a) Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation) ), which will take effect on 25 May 2018, I agree to the collection, storage and processing of my personal data provided by me to the controller.

Personal data processed includes name, surname, delivery address, e-mail address and telephone number. The given data will be used for sending the ordered goods. In case of request for sending news and notification of administrator, e-mail contact will be used for sending.

The controller will keep personal data for as long as necessary, for a maximum of 2 years.

Personal data will be processed by machines (automated) by means of computers, computer programs; event. in writing.


2) Data subject's rights, information and access to personal data

As a data subject, I have the right to access my personal data, have the right to correct or delete them, or limit processing. I have the right to request information about what personal data is processed and I have the right to request explanations regarding the processing of personal data.

Furthermore, I have the right to object to the processing as well as the right to data portability (ie the right to obtain personal data concerning me that I have provided to the controller in a structured, commonly used and machine-readable format).

I have the right to withdraw my consent at any time, without prejudice to the lawfulness of the processing based on the consent granted prior to its withdrawal, provided that the processing is based on Article 6 (1) (a). (a) or Article 9 (2) (a); (a) GDPR. This means that I do not have such a right, in particular, if the processing is necessary to fulfill the legal obligation to which the controller applies.

The controller declares that the processing does not involve the automated decision making, including profiling, referred to in Article 22 (1) and (4) of the GDPR. The controller declares that personal data is not processed for scientific or historical research purposes or for statistical purposes.

As a data subject, I declare that I am aware of (a) my rights under § 12 and § 21 of the Data Protection Act and my rights under GDPR.

I declare that all information provided is accurate and true and is provided voluntarily.

The controller declares that it will collect personal data to the extent necessary for the purpose and will process it only in accordance with the purpose for which it was collected.

This consent is a free and knowing expression of the will of the data subject, the content of which is the data subject's consent to the processing of personal data.
